All applicants for connection (including transfer, additional and change name) must bring Valid Identification (I.D.) Card (one of the following: SSS, PhilHealth, TIN, Voter's ID, Driver's License, Passport, Postal, Pag-Ibig, PRC ID, or employment's ID) to provide identity.

NO PROXY. Latecomers, children and youth below 18 years old are not allowed to attend PMES.
